Enhanced Care at Home Scheme supported by Warwick Charities

The King Henry VIII Endowed Trust, Warwick Relief in Need Charity, Warwick United Charities and Warwick Provident Dispensary Charity alongside the main contributor – the Charity of Thomas Oken and Nicholas Eyffler – and have supported this exciting initiative for the last 3 years

 

The funding allowed 3 Warwick GP practices led by Priory Medical Centre, and working with South Warwickshire NHS Foundation Trust, to offer enhanced and proactive care to their most vulnerable patients. Advanced Nurse Practitioners (ANPs) were recruited to enable the targeting of frail patients with advanced health needs, primarily in care homes, such as Priory View, to deal with issues before they necessitated hospital admission.

 

The 3-year project has provided considerable evidence that this approach to care provision provides a better service to patients and also saves the NHS money. Warwick Hospital have now committed to take on and fund the service, with a view to extending it over a wider area.
